    目的 探讨纤维支气管镜肺泡灌洗在呼吸衰竭治疗中的临床效果及安全性。方法 选取2023年10月至2025年8月郑州市中心医院收治的呼吸衰竭患者90例,按治疗方案不同分为对照组(n=44)和观察组(n=46)。对照组接受标准化常规治疗,观察组在对照组基础上联合实施纤维支气管镜肺泡灌洗。比较两组患者氧合功能[动脉血氧分压(arterial partial pressure of oxygen,PaO2)、PaO2/吸入氧浓度(fraction of inspired oxygen,FiO2)]、炎症指标[C反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)]、临床结局及不良事件发生情况。结果 治疗后,两组患者氧合功能及炎症指标均较治疗前改善,且观察组PaO2、PaO2/FiO2及CRP、PCT改善幅度均优于对照组(均P<0.05)。观察组机械通气时间、住院时间均显著短于对照组(均P<0.05),治疗有效率显著高于对照组(P<0.05)。两组不良事件发生率比较差异无统计学意义(P>0.05)。结论 在常规治疗基础上联合纤维支气管镜肺泡灌洗,可有效改善呼吸衰竭患者的氧合功能,减轻炎症反应,优化临床结局,且安全性良好。

    Abstract:

    Objective To evaluate the clinical efficacy and safety of fiberoptic bronchoscopy-guided bronchoalveolar lavage in the treatment of respiratory failure. Methods A total of 90 patients with respiratory failure admitted to Zhengzhou Central Hospital from October 2023 to August 2025 were enrolled and divided into control group (n=44) or an intervention group (n=46) according to treatment strategy. The control group received standardized conventional therapy, while the intervention group additionally underwent fiberoptic bronchoscopy-guided bronchoalveolar lavage. Oxygenation (arterial partial pressure of oxygen (PaO2), PaO2/fraction of inspired oxygen (FiO2)), inflammatory markers (C-reactive protein (CRP), procalcitonin (PCT)), clinical outcomes, and adverse events were compared between the two groups. Results After treatment, oxygenation and inflammatory markers improved in both groups, with significantly greater improvements in PaO2, PaO2/FiO2, CRP, and PCT levels in the intervention group compared with the control group (all P<0.05). The intervention group also showed shorter duration of mechanical ventilation and hospital stay (all P<0.05), as well as a higher overall treatment effectiveness rate (P<0.05). There was no significant difference in the incidence of adverse events between the two groups (P>0.05). Conclusion Fiberoptic bronchoscopy-guided bronchoalveolar lavage, when used as an adjunct to conventional therapy, can effectively improve oxygenation, reduce inflammatory responses, and optimize short-term clinical outcomes in patients with respiratory failure, with an acceptable safety profile.
