    目的 探讨基于健康行动过程取向(health action process approach,HAPA)模型的运动行为干预对2型糖尿病(type 2 diabetes mellitus,T2DM)患者运动依从性、自我效能及血糖控制的影响。方法 回顾性选取2023年1月至2025年10月在滁州市中西医结合医院管理的T2DM患者76例,根据干预方式分为对照组(n=38,接受常规糖尿病管理)和干预组(n=38,常规糖尿病护理联合基于HAPA 模型的运动行为干预)。干预前后对比运动依从性、运动自我效能及血糖指标[空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(glycated hemoglobin,HbA1c)]的变化。结果 干预后,干预组在运动频率、运动持续时间、行为坚持性及自我监督各维度的提升显著优于对照组(均P<0.05)。运动自我效能(情境、克服障碍及维持效能)改善程度亦优于对照组(均P<0.05)。血糖控制方面,干预组FPG 与HbA1c 降幅显著大于对照组(均P<0.05)。结论 基于HAPA模型的运动行为干预能够有效提升T2DM患者的运动依从性与运动自我效能,并进一步改善血糖控制。

    Abstract:

    Objective To investigate the effects of an exercise behavior intervention based on the health action process approach (HAPA) model on exercise adherence, self-efficacy, and glycemic control in patients with type 2 diabetes mellitus (T2DM). Methods A retrospective cohort of 76 T2DM patients managed at Chuzhou Integrated Traditional Chinese and Western Medicine Hospital from January 2023 to October 2025 was selected. Participants were divided into a control group (n=38, receiving standard diabetes management) and an intervention group (n=38, receiving standard diabetes care combined with HAPA-based exercise behavior intervention). Changes in exercise adherence, exercise self-efficacy, and glycemic indicators [fasting plasma glucose (FPG), glycated hemoglobin (HbA1c)] were compared before and after intervention. Results Post-intervention, the intervention group demonstrated significantly greater improvements than the control group in exercise frequency, duration, behavioral persistence, and self-monitoring dimensions (all P<0.05). Improvements in exercise self-efficacy (situational, obstacle-overcoming, and maintenance efficacy) also exceeded those of the control group (all P<0.05). Regarding glycemic control, the intervention group demonstrated significantly greater reductions in FPG and HbA1c compared to the control group (both P<0.05). Conclusion The HAPA-based exercise behavior intervention effectively enhances exercise adherence and exercise self-efficacy in patients with T2DM, further improving glycemic control.
