    目的 探讨基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS )技术在碳青霉烯类耐药肠杆菌科细菌(CRE )快速检测中的应用价值。方法 收集 2023 年 1 月至 2024 年 12 月临床分离的 CRE 可疑肠杆菌科菌株 104 株。PCR 检测 bla_ KPC 、bla_NDM 、bla_VIM 、bla_IMP 等基因,与 MALDI-TOF MS 检测结果进行对比。采集特征质谱峰并进行图谱分析,构建受试者工作特征(ROC )曲线评估诊断效能。结果 PCR 检出碳青霉烯酶阳性菌株 73 株(70.19%),MALDI-TOF MS 检测方法诊断灵敏度与特异度分别为 92.31% 、95.12% 。ROC 曲线下面积(AUC )达 0.880,Kappa 值为 0.873(P < 0.001 ),显示与 PCR 结果高度一致。结论 MALDI-TOF MS 能在短时间内实现对 CRE 的准确识别,具有操作简便、特异性强、时效性高等优势,为 CRE 早期筛查及耐药机制预警提供了检测途径。

    Abstract:

    Objective To evaluate the application value of matrix-assisted laser desorption ionization time-of- ?ight mass spectrometry (MALDI-TOF MS) in the rapid detection of carbapenem-resistant enterobacteriaceae (CRE). Methods A total of 104 suspected CRE strains were collected from January 2023 to December 2024. PCR detection of genes such as bla_KPC, bla_NDM, bla_VIM, and bla_IMP was compared with MALDI-TOF MS results. Characteristic mass spectrometry peaks were collected and analyzed, and receiver operating characteristic (ROC) curves were constructed to evaluate diagnostic performance. Results PCR detected 73 carbapenemase-positive strains (70.19%). The diagnostic sensitivity and speci?city of the MALDI-TOF MS method were 92.31% and 95.12%, respectively. The area under the ROC curve (AUC) reached 0.880, with a Kappa value of 0.873 (P < 0.001), indicating high concordance with PCR results. Conclusion MALDI-TOF MS enables rapid and accurate identi?cation ofCRE, o?ering advantages such as ease of operation, high speci?city, and timely results. This provides a detection pathway for early screening of CRE and early warning of resistance mechanisms.
